2: if your design requires stones - please indicate what type, shape, carat size and preferred quality you want to use OR if you are using stones you already have, please provide us with what the stone/s is, and the shape/size.

3: budget: what is your minimum - maximum range that you are comfortable spending?

4: delivery date: in order to best serve you, we need to know when you need your design by.

5: if you have any additional information or questions, please include them in notes.
